Changes for version 0.17

Add prefill method. Changed some statements so that cover could see they were tested. Removed some branches that wouldn't be executed to improve cover stats.

Generate HTML5 form label and input fields

To install Form::Diva, copy and paste the appropriate command in to your terminal.

cpanm Form::Diva

perl -MCPAN -e shell
install Form::Diva
