- Fixed a problem where by, if a directory was being viewed
in both the index and file manager mode by two different
browsers, the hidden files could appear in the index mode.
- Added IndexHtmlTag and IndexHtmlText directives to support
adding attributes and text for most HTML tags. This
includes <STYLE> support for casscading style sheets.

- Added three checks for $cfg-{ignore} existance.
- Added OpenIndexOptions UserDir directive and fixed ~user
processing for userdir_module.
- Added OpenIndexOptions UserSite directive. This option
allows a group of users to use the same GID which in
turn prevents each user from accessing other user sites.
- Added more IndexHtmlText directives to insert HTML text
just before each of the form text and submit objects.
