Changes for version 0.203 - 2011-06-13

Log and throw exception if a generated file can't be opened for writing. Fix class name generation to be consistent with wsdl2perl.pl so that everything except server has "s" appended to the name, instead of the other way around. Document default class prefixes, better error message if invalid prefix specified.
