Update how XML::LibXML::Parser is initialized to ensure that external entities are not loaded; looks like we can't assume that expand_entities is set to 1 on all platforms (Johann Rolschewski)
Do not try to read from filehandle that has reached EOF to avoid noisy warns. Issue pointed out by Mark Tompsett.
