Changes for version 0.13 - 2026-08-15

New device methods: connected_to(), connected_to_name() - provide info on the target device connected to in an open_port* call.
Minor doc, dist cleanups - repo/issue URLs should now be correct.
Cleaned up dependencies. Bump MIDI::Stream dependency to 0.006 for misc. bugfixes.
